You no longer need to complete M40 Application for invalidity benefits, D40 Application for invalidity benefit and superannuation productivity, and ADFC40 Application for invalidity benefits forms as the ADF Medical Transition process has changed.

Understanding your cover

When you join the ADF, if you’re eligible you’ll automatically get Death and Invalidity benefits through ADF Cover at no extra cost to you.
