Age retirement benefit application

Use this form to claim your PSS benefits as an age retiree. You can now complete this form online through your CSC Navigator account.

Preserved age retirement

Use this form to claim your preserved benefit as an age retiree. You can now complete this form online through your CSC Navigator account.

PSS election to cease membership form

Contributing PSS members can use this form to cease their PSS membership and join an alternate super fund.

Invalidity

Application for early access to preserved super benefits on medical grounds

This form is for PSS preserved benefit members who are seeking approval for early release / early access to preserved benefits on grounds of total and permanent incapacity.

Claim preserved benefit for approved invalidity

Use this form to apply for release of your preserved benefit as an incapacitated member unlikely to work again, if you have gained approval to access your preserved benefits on invalidity grounds.
